FEES & PAYMENTS
We make every effort to keep down the cost of your dental care. You can help by paying upon completion of each visit. Other arrangements can be made with our office manager depending upon special circumstances. We accept checks, Visa and Master Card. Extended payments and 12 month interest-free payments can be provided by Care Credit upon approval. If you have any dental insurance we will be glad to submit it for you. Our office tries very hard to estimate the patient’s co-pay. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. As your dental care provider, our relationship is with you, the patient, and not the insurance company. All charges are the patient’s responsibility from the date the services are rendered. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid for by your insurance company. You will be responsible for all collection costs, attorney fees, and court costs.
All balances over 30 days past due are subject to a 12% APR finance charge. Broken appointments and cancellations with less than 24 hours notice are subject to a $75.00 fee.
